NOTE: All California courts use the same basic set of forms. But some courts have special, local forms, too. To see if you will need any special, local forms, contact your court clerk or check your court’s website. The forms may be posted on their site. If not, the site will list the address and phone number of your local courthouse.

FORMS - Asking for a Spousal/Partner Support Order

Use the Information Sheet for Request for Order (Form FL-300-INFO) for information on how to fill out the Request for Order.


Ask your family law facilitator if you need to check the box for “Court Order” and item 4 on Form FL-300.

Income and Expense Declaration

FL-150

You need to attach:

  • A copy of your paystubs for the last 2 months; or
  • A recent profit and loss statement if you are self-employed or own any rental property.
Spousal or Partner Support Declaration Attachment  FL-157 Use this form when asking to change a spousal or partner support order made as part of the divorce or legal separation judgment, or after the judgment.

This is an optional form, so you do not have to use it but it can help you make sure you give the judge the information s/he will need.
