You may not need all of these forms. Or you may need more forms. If you are not sure which forms to use, talk to your family law facilitator, self-help center, or a lawyer. Click for help finding a lawyer.

NOTE: All California courts use the same basic set of forms. But some courts have special, local forms, too. To see if you will need any special, local forms, contact your court clerk or check your court’s website. The forms may be posted on their site. If not, the site will list the address and phone number of your local courthouse.

Judgment (Family Law) FL-180 Cover sheet with attachments that you prepare on blank paper stating the judgment terms or form attachments. If you and your spouse or domestic partner have signed a notarized settlement agreement or stipulated judgment, attach it to Form FL-180.

If you (the petitioner) are asking for child custody or visitation orders, also fill out and file the following form:

Attach a copy of your pay stubs for the last 2 months or a recent profit and loss statement if you are self-employed or own rental property.

Spousal or Partner Support Declaration Attachment FL-157 This form is optional but it helps you make sure you give the judge all the information he or she will need, according to the law, to make a decision about spousal or partner support.
Spousal, Partner, or Family Support Order Attachment (Family Law) FL-343 Lists the spousal, partner, and family support orders you want.

Notice of Rights and Responsibilities — Health-Care Costs and Reimbursement Procedures and Information Sheet on Changing a Child Support Order FL-192

Explains how to handle your children’s health-care costs.

Explains how to change the support order in the future.

If you have a child support order, you should read and must attach this form to your judgment form, Form FL-180.
Child Support Case Registry Form FL-191 Registers the case in a national registry to help with enforcement. Instructions are included with this form.
Earnings Assignment Order for Spousal or Partner Support (Family Law) FL-435 Used to order the paying person’s employer to withhold spousal/partner support from the paycheck and send it to the other spouse or domestic partner.

Use this form if you are asking for ONLY spousal or partner support.

When filling out Form FL-435, make sure to only write the last 4 digits of the social security number of the person ordered to pay support – the law requires it to protect their privacy.

Income Withholding for Support FL-195 Used to order the paying person’s employer to withhold child and spousal or partner support from the paycheck and send it to the other spouse or domestic partner.

Tells the court your spouse or domestic partner (the respondent) is making an “appearance” in the case and agrees to waive certain rights and go ahead as an uncontested case. The respondent must sign it.
